Nosocomial infections in neonates caused extended-spectrum beta-lactamases (ESBL) producing Klebsiella pneumoniae (ESBL-KP) are associated with severe outcomes. We investigated, using whole genome sequencing, three parallel ESBL-KP outbreaks in different neonatal intensive care units (NICUs) in close geographical proximity. In all three outbreaks, we found a dominant clone which spread among neonates, as well as satellite clones occurring sporadically. Three different dominant clones were detected, each responsible for an outbreak in one NICU (NICU-1, ST985; NICU-2, ST37; and NICU-3, ST35). In both NICU-1 and NICU-2 blaCTXM-15 was the ESBL gene carried by the dominant outbreak clones, and in both, it was carried on the exact same transposable element. In NICU-3 the the dominant clone carried a blaCTXM-14.The virulence genes content of the dominant clones in NICU-1 and NICU-2 and of a satellite strain from NICU-3 (ST268) were similar. These strains. carried yersinibactin (ybt) genes of different types and sources; ST985 acquired ybt16, ST37 was found to code for variant ybt5 and ST268 for ybt10 all on different integrative conjugative element types. to All three dominant clones coded for an additional iron uptake system which is carried on an IncFII-IncFIB plasmid along with heavy metal and osmotic stress resistance genes.
