host descriptionPatient with recent cervical fracture, status-post laminectomy and tracheostomy, presented from subacute rehabilitation for evaluation of septic shock. Workup revealed acute respiratory failure secondary to staphylococcal bacteremia with new sacral ulceration as presumed source. Clinical course was complicated by a cervical spine hematoma extending into the interspinous space, ligamentum flavum disruption secondary to same, superventricular tachycardia refractory to cardioversion, acute kidney injury with acute tubular necrosis in the context of active urosepsis, severe protein calorie malnutrition and acute-on-chronic anemia secondary to hematuria. Patient was managed medically with cefazolin to be continued on discharge. Patient was a poor candidate for spinal surgery and could not receive transesophageal echocardial imaging due to spinal injury. Patient was stabilized and discharged to subacute rehabilitation facility. Duration of bacteremia: 4 days.
