host descriptionPatient with end-stage renal disease on hemodialysis presents with septic shock related to her tunneled hemodialysis line which was removed. Clinical course was complicated by encephalopathy, acute hypoxic respiratory failure and upper gastrointestinal bleeding. Medical management consisted of vancomycin with cefepime, ceftriaxone, ampicillin and acyclovir added for empiric meningitis coverage. While hopsitalized, the patient developed a myocardial infarction refractory to cardiac revascularization. In the setting of unstable, refractory ventricular arrhythmias on maximum pressor support, the patient's code status was changed and she expired. Duration of bacteremia: 4 days.
