host descriptionPatient with chronic encephalopathy presents on transfer from outside hospital with mental status changes and a displaced epigastric tube. Blood cultures grew methicillin-resistant Staphylococcus aureus managed with vancomycin. Clinical course was complicated by severe calorie malnution, septic versus cardiogenic shock, tension pneumothorax, acute hypoxic respiratory failure requiring emergent intubation and pressor support and bilateral deep vein thromboses in the setting of low hemoglobin. Patient developed coagulopathies, acute kidney injury and remained hypotensive on triple pressor therapy. Patient developed pulseless electrical activity and expired. Duration of bacteremia: 4 days.
