NOTES: Percentages are based on a sample of 3,807,097 visits by adults to 58 health centers, representing an estimated 88.6 million adult visits (weighted) to health centers in all 50 U.S. states and the District of Columbia. Health centers at which all diagnoses were missing for all visits (n = 6) were excluded, and visit weights for the remaining health centers were normalized to account for excluded ones. Health center visits were categorized into diagnosis chapters using the International Classification of Diseases, 10th Revision, Clinical Modification (ICD–10–CM) codes that were recorded in electronic health records. Visits can be represented in multiple diagnosis chapters. The total exceeds 100% because more than one diagnosis can be reported per visit. ICD–10–CM chapters P00–P96 and Q00–Q99 are not included in the table because they represent diagnoses more frequent in children than adults. Sex was missing for 0.1% of the visits by adults, and missing values were excluded from the denominator for the subgroup analysis by sex.
